Check if the outputs are referenced twice.

Possible that the second ladder is overriding the first ladder and then you start to have weird things happening like what you are seeing.

No. All outputs only in one place.
In "Chart Status" signals are OK, The condition match with the real outputs. Not like in online blocks.
